Cefixime and Ofloxacin Tablets

Category: Combination antibiotic therapy (Cephalosporin + Fluoroquinolone)
Form: Oral tablets

Uses:

  • Broad-spectrum treatment of bacterial infections, particularly when mixed or resistant organisms are suspected.

  • Common indications:

    • Respiratory tract infections (bronchitis, pneumonia, sinusitis)

    • Urinary tract infections (UTIs)

    • Gastrointestinal infections (including typhoid fever, infectious diarrhea)

    • Skin and soft tissue infections

    • Gynecological infections

Mechanism of Action:

  • Cefixime: Third-generation cephalosporin

    • Inhibits bacterial cell wall synthesis, leading to cell death.

    • Active mainly against Gram-negative bacteria and some Gram-positive bacteria.

  • Ofloxacin: Fluoroquinolone antibiotic

    • Inhibits bacterial DNA gyrase and topoisomerase IV, enzymes essential for DNA replication.

    • Active against both Gram-positive and Gram-negative organisms.

  • The combination provides wider antibacterial coverage and may help in resistant or mixed infections.

Administration:

  • Taken orally, usually once or twice daily, depending on infection severity.

  • Can be taken with or without food, but maintaining the same timing each day is recommended.

  • Duration typically ranges from 5–14 days, depending on the type of infection.

Side Effects:

  • Common: nausea, vomiting, diarrhea, abdominal pain, headache, dizziness.

  • Other: mild skin rash, insomnia.

  • Rare but serious:

    • Allergic reactions (rash, swelling, breathing difficulty)

    • Tendonitis or tendon rupture (associated with fluoroquinolones like ofloxacin)

    • Severe diarrhea due to C. difficile infection

    • Nerve-related issues (tingling, numbness)

Precautions:

  • Avoid in patients with allergy to cephalosporins or fluoroquinolones.

  • Use cautiously in patients with kidney or liver disease.

  • Ofloxacin may cause dizziness or lightheadedness; avoid driving or operating heavy machinery if affected.

  • Should not be used in children, pregnant or breastfeeding women unless clearly prescribed, due to fluoroquinolone-related risks.

  • Complete the entire course to prevent resistance.

Summary:
Cefixime and Ofloxacin Tablets combine two antibiotics — a cephalosporin (cefixime) and a fluoroquinolone (ofloxacin) — to provide broad coverage against bacterial infections. This combination is effective in treating respiratory, urinary, gastrointestinal, and skin infections, as well as certain resistant or mixed infections. While effective, it must be used with caution due to possible side effects like digestive upset, tendon injury, and allergic reactions.
