Daunorubicin Injection

  • Type: Chemotherapy medication

  • Drug Class: Anthracycline antibiotic (antineoplastic agent)

  • Form: Sterile solution or powder for reconstitution, given intravenously (IV)

How It Works

  • Daunorubicin works by intercalating into DNA (slipping between DNA strands) and inhibiting topoisomerase II, an enzyme that helps unwind DNA during replication.

  • This blocks DNA and RNA synthesis, preventing cancer cells from dividing.

  • It also produces free radicals, which damage cancer cell DNA and cell membranes, leading to cell death.

Common Uses

Daunorubicin is mainly used to treat blood cancers:

  • Acute Myeloid Leukemia (AML) – as part of induction chemotherapy

  • Acute Lymphoblastic Leukemia (ALL) – in some treatment regimens

  • Other leukemias – in selected cases

Advantages

  • Highly effective at inducing remission in AML and ALL

  • Often combined with cytarabine (the “7+3 regimen”) for better results

  • Rapidly kills rapidly dividing leukemia cells

Possible Side Effects

Daunorubicin has some specific side effects that require monitoring:

Common

  • Bone marrow suppression (low blood counts, risk of infection, anemia, bleeding)

  • Hair loss (alopecia)

  • Nausea, vomiting, mouth sores

  • Red coloration of urine for 1–2 days after dose (harmless but expected)

Serious

  • Heart toxicity (cardiotoxicity) – risk of heart damage increases with cumulative dose

  • Tissue damage if drug leaks outside vein (extravasation) – can cause severe local injury

  • Liver function abnormalities

  • Rare: secondary leukemia with prolonged use

Precautions

  • Heart function (e.g., echocardiogram) is checked before and sometimes during treatment

  • Maximum lifetime cumulative dose must not be exceeded to reduce heart failure risk

  • Administered only through a properly placed IV line by trained professionals

  • Blood counts and liver function monitored closely
